Where the Complainant is a minor (not having reached the age of 18), parental consent is required in order that FIDReC can administer and deal with the complaint (“FIDReC’s proceedings”).

  1. Both the Complainant and the Complainant's parent/ guardian MUST read this document carefully and thoroughly and MUST sign on the last page of this document and return the same to FIDReC together with the following:
     
    A copy of the Birth Certificate of the Complainant has to be submitted to FIDReC,

    AND

     
  2. If the consent is given by the parent, a copy of the NRIC of the parent has to be submitted to FIDReC,

    OR

     
  3. If the consent is given by the guardian, a copy of the court order or will or deed that appoints the guardian under Section 7 of the Guardianship of Infants Act has to be submitted to FIDReC as well as a copy of the NRIC of the guardian. In the case of a will appointing the guardian, a copy of the Grant of Probate has to be submitted to FIDReC.
