Where the Complainant is a minor (not having reached the age of 18), parental consent is required in order that FIDReC can administer and deal with the complaint (“FIDReC’s proceedings”).
- Both the Complainant and the Complainant's parent/ guardian MUST read this document carefully and thoroughly and MUST sign on the last page of this document and return the same to FIDReC together with the following:
A copy of the Birth Certificate of the Complainant has to be submitted to FIDReC,
AND
- If the consent is given by the parent, a copy of the NRIC of the parent has to be submitted to FIDReC,
OR
- If the consent is given by the guardian, a copy of the court order or will or deed that appoints the guardian under Section 7 of the Guardianship of Infants Act has to be submitted to FIDReC as well as a copy of the NRIC of the guardian. In the case of a will appointing the guardian, a copy of the Grant of Probate has to be submitted to FIDReC.
