Authorisation Form

If you are filing a complaint related to a joint account, all account holders/ policyholders/ insured parties must complete the Authorisation Form before FIDReC can handle your complaint. This ensures that we have the consent of all parties involved to file the complaint with FIDReC. 

The Complainant and all joint account holders/ policyholders/ insured parties must read and sign the Authorisation Form, and upload the completed form to the case submission portal along with a copy of their NRIC/ FIN/ Passport.


Parental Consent Form

If the Complainant is a minor (under 18), parental consent is required for FIDReC to handle the complaint. Both the Complainant and their parent/guardian must read and sign the Parental Consent Form and upload the completed form to the case submission portal along with the following:

1.    A copy of the Complainant’s birth certificate

2.    If the parent gives consent, a copy of the parent’s NRIC/ FIN/ Passport

3.    If the guardian gives consent, a copy of the court order/ will/ deed appointing the guardian under Section 7 of the Guardianship of Infants Act, a copy of the guardian’s NRIC/ FIN/ Passport, and, in a case of a will appointing the guardian, a copy of the Grant of Probate
