If you require the assistance of a nominee for your case, please complete and sign a copy of the Nominee Appointment Form and the Nominee Confidentiality Agreement, together with your nominee, and submit it to the case manager of your complaint to formalise their appointment. Please note that if your nominee is a lawyer, they should not be receiving any monetary benefit from their appointment as your nominee.
